Infliximab is used in patients with inflammatory rheumatic diseases whose inflammations have responded inadequately to conventional treatments.

The drug is also used in inflammatory bowel disease. For Crohn’s disease from a patient’s age of six years, if the course despite the usual treatment is difficult and complicated. Also, ulcerative colitis can be treated with infliximab if other therapies were ineffective.

Infliximab also has an anti-inflammatory effect on ankylosing spondylitis and is used when other antirheumatics have not worked well.

Effect of Infliximab

Infliximab belongs to the group of genetically engineered immunological antibodies. The active ingredient acts against the tumor necrosis factor TNF alpha (TNF alpha antagonist). TNF alpha is an endogenous substance that produces the immune system for the destruction of foreign cells and proteins. In some diseases, such as Crohn’s disease, a chronic inflammatory bowel disease, but also endogenous cells are destroyed by TNF alpha. This cell-destroying effect is blocked by the active ingredient.

Together with interleukin, TNF alpha plays a major role in the activation of phagocytes (macrophages) and certain white blood cells (lymphocytes). These are necessary for the defense reaction in case of inflammation.

The active ingredient binds to TNF alpha and together forms a stable antibody complex. The disadvantage is that this process also weakens the body’s defense reaction against infections and cancer cells.
