Metoprolol is used in the treatment of hypertension, which has no organic causes (essential hypertension), and cardiovascular problems.

In addition, metoprolol is prescribed for heart disease associated with inadequate oxygenation of the heart muscle (myocardial insufficiency) and circulatory disorders of the coronary heart disease (coronary heart disease). In this context, metoprolol is used in stable, not causing discomforting angina pectoris.

Another area of use of the drug is cardiac arrhythmia associated with too rapid a heartbeat. Metoprolol can have a beneficial effect on these cardiac arrhythmias because it slows down the heartburn.

Metoprolol continues to be used in the acute treatment of heart attack to relieve the heart. After a heart attack, the drug is used preventively in long-term treatment to reduce the risk of further incidents leading to sudden cardiac death.

Mode of action of Metoprolol

The active substance metoprolol is a beta-blocker with so-called beta-1-selective properties. This means that it acts primarily on the beta-1 receptors. Beta-1 receptors are special binding sites of the sympathetic fibers of the autonomic nervous system, which are normally activated by the body’s own messengers adrenaline and norepinephrine. Metoprolol displaces these endogenous messenger substances at the receptors and thus suppresses their effect.

At the beta-1 receptors of the heart, the drug slows down the transmission of electrical impulses to the conduction system. This lowers the number of heartbeats and the amount of blood pumped during each heartbeat (heartbeat volume). As a result, the blood pressure drops significantly and the heart is calmed and thus relieved when the stress level is too high.

At beta-1 receptors in the kidneys, metoprolol cushions the release of the blood pressure enhancing hormone renin. So the blood pressure is additionally lowered.

The efficacy of metoprolol in preventing migraine attacks is directly attributed to inhibition of beta-receptors in the brain but is not fully understood.

Metoprolol is almost completely taken from the gastrointestinal tract after taking it as a tablet. However, metoprolol is subject to a pronounced so-called “first-pass effect”. This means that about half the amount of active ingredient is already degraded in the liver, before it is distributed through the bloodstream in the body and the site of action, for example to the heart, and can act there.     I have been taking Metoprolol Retard 100 for 3 weeks and have been struggling with various side effects for a week. Tachycardia, headache, swollen ankles and feet, sleep disorders, anxiety and profuse sweating (I am already panicking when I have to wait somewhere).     First 2 weeks only 25mg to get used to. Then increased to 50mg. Result was after a few days very strong, even on day disturbing tinnitus. Self-reduced to 25mg, slight improvement in symptoms. After check-up at the doctor have been put on new drug (ACE inhibitor – lisinopril).
