Application of the active substance sitagliptin

In patients with type 2 diabetes mellitus, the drug is used to improve glycemic control. Sitagliptin can be used alone or in combination with other oral antidiabetics (metformin, sulfonylureas, glitazones).

Single therapy with sitagliptin is used when the active substance metformin can not be used. In dual therapy, the drug may be used with metformin, a sulfonylurea or a glitazone. Also a triple therapy together with metformin and a glitazone is possible. A prerequisite for the addition of sitagliptin is an inadequate lowering of blood sugar through diet and exercise plus monotherapy with glitazone, metformin, a sulfonylurea or the dual therapy of metformin and sulfonylurea.

Sitagliptin is also approved as adjunctive therapy to insulin in the treatment of patients with type 2 diabetes. This will allow the use of the drug alone or in combination with metformin in addition to insulin if diet and exercise plus a stable dose of insulin and metformin are not enough to lower blood sugar.

How Sitagliptin works

Sitagliptin belongs to a new subgroup of oral antidiabetics called dipeptidyl peptidase inhibitors. The substances of this group inhibit an enzyme that affects the blood concentration of glucose.

The blood sugar level (blood glucose concentration) is controlled by a control circuit in which so-called incretin hormones play a major role. Incretin hormones are released from the intestine into the bloodstream throughout the day and their concentrations increase in response to a meal. When the blood glucose concentration is normal or elevated, the incretin hormones promote the production and release of insulin from the beta cells of the pancreas. More insulin, however, promotes glucose uptake into the tissue. At the same time, the incretins throttle the liver’s glucose production. All effects taken together cause the blood sugar level to drop. These effects of the incretin hormones are glucose-dependent, so that at low blood sugar levels neither a release of insulin nor a reduced glucose production of the liver occurs. Therefore, incretin hormones can not trigger hypoglycaemia.

The activity of the incretin hormones is limited by the enzyme dipeptidyl peptidase. It quickly breaks down the incretins into inactive products. Sitagliptin prevents enzyme-induced degradation of the incretins. By increasing the concentration of active incretin hormones, sitagliptin increases insulin release and lowers blood glucose levels.

In type 2 diabetics, these changes result in lower blood glucose levels both in the fasting state and after meals.

    I take Janumet 50/850. Before that 50/1000. Both for about 2 years once in the morning and once in the evening. Side effects: – severe diarrhea, actually since taking no more normal bowel movements – bloated lower abdomen / intestinal area – sweats associated with the stomach complaints – listlessness Janumet has managed to lower the BZ, however, the side effects are very annoying and restrictive in everyday life.

    I have tolerated the Janumet very well, but got a bloated stomach, last had a fasting value of 140, Hbc1 value of 6.6 and the diabetes doctor has switched to Januvia 100mg and forxgia5mg, the bloated stomach is gone, but the values are extremely high sober 170 , 180, but the Abblähbauch is gone.     Too much wheat beer !! Am now after a week after diet back to about 150 (long-term value was 280 and peak was 540!). I take Janumet 50 every morning for breakfast and before going to bed. Despite low calorie diet and zero beer, my overweight is unchanged. If I omit Janumet, then the sugar value rises around 20-30. Side effect may have been a tight belly, take OMEP now. The effect of Janumet 50 seems to be to prevent the liver from giving off glucose, especially at night. A brisk walk before going to bed causes miracles, but I do not do that in wind and weather. The regular measuring of the blood sugar and the ambition helped to bring it down again.

    I have been taking Janumet for 3 years as a replacement for metformin and insulin. The blood counts were good. After about 2.5 years I got itching on the arms, then on the legs and the whole upper body. Due to the inevitable itching I had infected areas everywhere. The 3rd Dermatologist then decided for the diagnosis Neurodermatitis . So at age 64, I should have got eczema. (Incidentally, hay fever since the beginning of this year). The diagnosis left me no peace. That’s why I had my medication tested for compatibility in a lab. It was investigated LTT and BDT. As a result, the Janumet BDT (basophil degranulation test) value was found to be 1,189 pg / ml. The reference range is less than 200. That means type I sensitization. I stopped the drug immediately and have been taking metformin for a week now. I have the feeling that the itching is now subsiding.

    in connection with Capozide my partner suffers from nausea, dizziness and the worst thing is the itching. Today, Janumet’s internist has switched back to Diabetex 500 and will wait 3 weeks to see if the itching subsides.

    Glimepiride in combination with Siofor had previously been prescribed 1000 mg (one in the morning and one in the evening). – Unfortunately, the Glimepiride caused me but a steady and (in total) from me no longer tolerated weight gain, which is why my doctor then on my strong request after 6 months also (finally) discontinued. – With the Siofor alone, I was pretty good, no side effects, but the values were unfortunately not quite in the normal range, which is why I finally got Janumet, which I have been taking for 3 months now. – Weight loss in this time about 10 kilograms, but I personally write this less the Janumet, as the deposition of glimepiride. – For about 4 weeks, my weight stabilized at 91 kilos (179 cm height) and falls no further, which confirms me in my assumption only. – Side effects of Janumet are clearly mild diarrhea, but not all the time.     Have previously taken gylmprid with medformin, which could lead to the hypoglycaemic manifestations were due to gylmprid.
