Mode of action of Enalapril

Enalapril belongs to the group of ACE inhibitors. The blood pressure reduction of ACE inhibitors occurs in three ways:

  • Blockade of the enzyme ACE (Angiotensin Converting Enzymes) prevents the formation of the hormone angiotensin II in the kidney. Angiotensin II causes a strong contraction on the blood vessels and thus an increase in blood pressure. If ACE is blocked by the ACE inhibitors, practically no more angiotensin II can be formed. The blood vessels relax and the blood pressure drops.
  • Increased salt and water are excreted through promotion of kidney function. This reduces the blood volume and lowers the pressure in the vessels.
  • by direct dilation of the blood vessels by inhibiting the degradation of the vasodilator bradykinin. As a result, bradykinin can work longer, and the blood vessels stay longer.

Enalapril is actually a prodrug: it is itself ineffective and is only converted to its active form enalaprilat in the liver. The gift as a prodrug has the advantage that the active ingredient is better absorbed into the body as its effective conversion product. However, the effect of enalapril is delayed by the conversion. The effect begins approximately one to two hours after ingestion, but lasts for 18 to 24 hours.

Enalapril, or enalaprilat is mainly excreted via the kidneys. Therefore, the doctor must reduce the dose in patients with severe renal impairment and deficient urinary output because the effect is stronger and lasts longer.

    I take medication with enalapril and metoprolol for 18 years. The effect is excellent, though over the decades I have seen a steady increase in the dose.
