Application of the active substance methylphenidate

The drug methylphenidate is used in attention deficit hyperactivity disorder, which is referred to as attention deficit hyperactivity disorder (ADHD) in children over the age of six years.

The attention-deficit and hyperactivity disorder manifests itself through lack of concentration and impulsivity with marked hyperactivity and concomitant symptoms such as increased irritability, ill-humor or aggressiveness, depression and forgetfulness.

ADHD does not cure and persists in approximately 60 percent of those affected into adulthood. Adults with ADHD mainly show restlessness, impatience and inattention. Overactivity may decrease with age due to adaptation, development of the nervous system, and treatment with over-the-counter medications. Often there is an inability to social bonds. Today ADHD is also treated with methyphenidate in adults.

According to recent research, the cause of ADHD is disrupted signaling in the brain. Since ADHD is a very complex clinical picture with very different symptoms, treatment with the active substance methylphenidate can only ever be a therapeutic component within a so-called multimodal treatment plan consisting of several building blocks or ways. With careful dosing by the doctor, however, the administration of methylphenidate has a very decisive positive influence on the main symptoms of the disease. The active ingredient reduces inattention and distractibility as well as impulsivity and hyperactivity.

A second indication of methylphenidate is narcolepsy in adults. This is a disturbance of the sleep-wake rhythm, which is expressed by excessive daytime sleepiness with sudden onset of sleep. The sleep attacks of narcolepsy, sometimes triggered by extreme stress, have nothing to do with lethargy and therefore can not be resolved with a lot of sleep. In addition to the main symptom of the disease other malfunctions such as excitation caused by occurring short-term loss of muscle tension (cataplexy), daydreams (hypnagogic hallucinations), sleep paralysis and disorders of nighttime sleep.

Also, the causes of narcolepsy are not well known. However, it is believed that disruption of the part of the brain responsible for sleep / wake control is responsible for the onset of the disease. Narcolepsy is still not curable. The daytime fatigue can be mitigated by methylphenidate, while cataplexy and sleep paralysis with tricyclic antidepressants and also the disturbed nighttime sleep must be treated separately.

Mode of action of methylphenidate

Methylphenidate belongs to the group of alpha sympathomimetics with stimulating effects on the brain.

The drug reduces distractibility in attention-deficit hyperactivity disorder (ADHD). How exactly this happens is not yet finally understood. There are no fewer than four different explanatory approaches. The neurotransmitters norepinephrine and dopamine, which provide for concentration and categorization of information in the brain, may play a role. Among other things, methylphenidate could prevent too fast uptake of the messenger substances and thus ensure that there is no lack of norepinephrine and dopamine in the brain. As a result, the brain could better perceive its control functions.

Observable effects of methylphenidate include decreased motor hyperactivity (“Zappelphilipp syndrome”) and decreasing impulsivity in children with ADHD. Due to the improved impulse control tasks can be completed better and the aggressiveness can be mitigated.

    My son, who has been treated for ADHD for some time, should be given Concerta 18 mg Retard (note that this is the lowest dose) as it can be taken before breakfast, faster and longer. In the beginning, i. for about 6 months we were also quite satisfied with the drug, the loss of appetite was less strong than the old drug, but the sleep problems were getting worse. At some point we were at 23 o’clock in the evening. But the real nightmare started after 1 year: my son became more aggressive every day from 4pm (end of effect). He screamed and raged every afternoon for 4-5 hours. Sometimes I was to blame, sometimes he hit his head, with the words that shit head should finally stop thinking, then he became self-aggrandizing, hit his head against the wall, cowering under the bed, wanted to die (with 10 years!) I was totally desperate, ran from doctor to doctor. It was agreed: something can only lie on the mother, the child must go to the home! I was stunned !!! At some point my feeling told me to stop using these tablets. They do not help anymore … and the miracle happened! Already 1 day (!) After discontinuing Concerta, the spook was over. Instead of congratulations I got anger from the doctor, because of arbitrary action, but …, my son may stay, we lead a better life again. He has rediscovered his joy in life. Meanwhile, a mother has reported similar to me. My opinion: Stay away from Concerta !!!

    Concerta for Adh syndrome

    I have been taking Concerta for over 2 months. Originally I wanted to take it because of my ever-worsening lack of concentration during a somewhat tough semester and then sell it again. However, I can not just focus on Concerta, I also realized that I am balanced, no longer afraid of other people, no longer running around the world as if dazed, finally having motivation to do something. I am resilient and constantly calm with a positive attitude. In addition, I have the feeling that I finally manage to do what I always do. Also the appointment with the psychiatrist, who has finally confirmed to me ADS. I’m not going to drop Concerta anymore, I’m fine too. I finally feel normal. Side effects include loss of appetite (often eaten from nervousness in the past) with slight weight loss, insomnia at the beginning of intake and sometimes a small rebound effect in very stressful times, which manifests itself in the morning before taking.     My experience in the beginning was relatively still okey. But I realized at the age of 17, that I do not like to volunteer and would do it voluntarily.     My son is 11 years old and has been getting concerta for 2 years. We have had very good experiences. Social behavior and concentration have changed very much for the better. Without the drug, he would not have made a transition from elementary school to high school, although an IQ test revealed that he is more than intelligent in the natural sciences.     I’m in my 30s, female and have ADHD. Now I finally wanted to try a retard drug – and have Concerta prescribe me. Result: I did not notice any effect at 18mg except that I was even more aggressive than WITHOUT medication. Normally I notice with 3x5mg methylphenidate (Hexal) a good effect! Conclusion: Concerta, no thanks! So much annoying to my family as with Concerta I’ve never been – most of all my little daughter had to suffer from me!

    I take CONCERTA 36mg for almost 2 years and 150mg Wellbutrin for one year. In between, I increased the CONCERTA, which was too much for me, because I felt that my body was completely overwhelmed with the whole medis. Incidentally, I still take Premens, so that the hormonal fluctuations are not so strong, which has a very positive effect on me. I recently discontinued the Wellburtrin because I was not sure if I needed it at all and what exactly was the positive about it. The sharp pain that I had before the Wellbutrinphase, but came back and so I started to take the Wellbutrin again. So what can I do against my situation? Although I am more concentrated, but the concentration has the side effect that I sometimes feign in a thing verzeschel and no longer come out.     In my experience, I can say that taking Cocerta Retard helps me lead a much better life. The quality of life is much higher, you can do more and above all, the concentration is much better. I’ve had good school results since taking this drug. Learning was easier for me. In general, I feel better, the side effects are very low with me, only often fatigue and sometimes nausea. As you know, there are NO LONG TERM STUDIES !!! i.e. No one knows what can happen after a 7 years ‘take … That’ s why today I would like to tell you how I am now after the dismissal … In June 2008, I was 19 years and since the cash only until the end of I had decided to sell Concerta for a long time and we had a great deal of physical and mental well-being … In November I gained 10kg within 2 weeks, though I’m 6km to the university and am not a big eater, until today I bring 20kg more on the scales … because of the weight gain I also have problems with my ankle.The increase comes from the way my increased cortisol levels.But the worst was In January, erythema nodosum, nodule in the lymphatic system (hurt a hell of a lot, and unstable patients probably jumped out of the window Hormones Roulette with me … Oh, and my performance got worse as the disease progressed. So far, all the doctors have lost their teeth on me … First, it was called pituitary adenoma, which could be refuted. Now my adrenal cortex is still being studied because most of the crazy hormones are formed there. And if all this does not work … Plan B Concerta ???     I have got concerta 27mg described by the doctor. Since I use it I have a headache Abdominal pain I feel sick I have to vomit at the beginning I still did not know it was due to the drug but yesterday the headache was so bad I started screaming I was sick and my eyes were blazing red pupil too big i looked blurry my father took me to artz’s hospital could not tell me what it was because I did not tell him about the drug because I did not think it at the moment it was due to the drug when I was home I realized that it was the concerta I googled the side effects and was shocked most of what I had. I do not know if och the drug should continue to take me I use it only because of my lack of concentration I went to the pharmacy he said that I should not take it is too dangerous so many side effects only for the concentration jz I do not know what I am so it helped me a little in concentration and i’m not frustrated hungry anymore what i found good i take too much too fast and cant lose my weight and i can always better listen in school come with only the side effects getting me doubt if you could help me I would be happy 🙂

    Side effects with both drugs no. I only take Concerta for a month, and I’m not sure if the low dose is enough. But with the current requirements, which are put to me, it is sufficient, so I get along well with everything. Had 3 times daily 10g Methylphenidat. At any rate, I feel better now. Venlafaxine (since 3 months) is unlikely to help in such a low dose. But I was suddenly thirsty for a glass of wine in the evening. Also, the search for chocolate or other mood brighteners was completely. So does it help some patients? Especially since I found the hint in an ADHD book.
